We are building on our locality and partnership work as part of a wider Alliance, to keep the residents of Suffolk at the centre of what we do and improve their outcome.
It is made up of statutory health and social care providers and commissioners, representatives from the voluntary and community sector and representatives from District and Borough Councils; with the aim of:
- delivering health and care which uses resources more flexibly and effectively
- carrying out more preventative work to help people to stay well, and more independent for longer
- providing more co-ordinated, holistic and preventative care
- empowering people to play a central role in managing their own health and wellbeing.
The Locality and Team
ACS operate in three localities (Ipswich and East, West and North) covering Suffolk, each with an area office based in a main town.
The Ipswich and East Locality has been in the vanguard of joint management social care and health posts which are now well established, with four already in place. This approach is multi-disciplinary and your role will be to lead and manage the whole Integrated Neighbourhood Team (Community Health and Social Care).
You will lead and manage the whole South Rural (INT) Integrated Neighbourhood Team; which brings together both Community Health and Social Care, to provide the right mix of support in a more efficient and timely manner to meet people’s needs.
This will see you contribute to the effective leadership of ACS, as part of the Ipswich and East Management Team delivering good quality services and effective integration between health and social care , collaborating with stakeholders and partners across the system leading to better outcomes for the people of Suffolk.
Managing alongside key partners, system leadership will be a key part of the role to ensure health and social care systems are joined up and accessible for Suffolk residents.
As part of Ipswich and East management team, you will support the Operations and Partnerships Managers, working with Area Directors, County Councillors, the Director of Adult Social Care as well as health leaders and managers to deliver the council’s political and strategic agendas.
