Salary: £20,903 pro rata for part time

We are looking to recruit an Assessor across our service in Home First Ipswich & East.

Home First Ipswich & East are an in-house care and reablement service which assists people to regain and maintain their independence, allowing them to remain living in their own home for longer.

You will take a lead on the effective delivery of customer flow and outcomes across health and social care by designing effective care and reablement plans and reviewing customers in a timely manner. You will be based in the community under the locality and work closely with our health partners to identify reablement customers.

You will have:

  • a passion for improving the lives of our customers 
  • excellent communication skills across multiple stakeholders 
  • quality recording skills.

This organisation positively encourages the use of technology to communicate and engage, but in this role you will need to operate across a wide and rural area, so you must either hold a full and current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.
