We have an exciting opportunity to join the Complex Hospital Team at Ipswich Hospital to support people with safe and effective discharge planning. We are looking for an enthusiastic person with a positive attitude.

We offer a supportive team and a chance to work as part of a busy integrated team with other health professionals.

You will:

  • play a key part in promoting personalisation, independence, wellbeing and choice by providing a social care service for people
  • exercise a high level of professional judgement and risk evaluation, taking responsibility for the safety and well-being of people accessing adult and community services within a health environment
  • complete asset-based assessments / reviews, short term reablement plans, care and support plans and safeguarding plans in line with Supporting Lives, Connecting Communities and using a Signs of Safety and Solution Focused approach.

This post will include a rota of once a month Saturday working.

This organisation positively encourages the use of technology to communicate and engage, but in this role you will need to operate across a wide and rural area, so you must either hold a full and current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.
